What is a Hair Follicular Unit?

Your hair is divided into sections called hair follicular units. A hair follicular unit is a naturally occurring group of hairs that consist of 1-4 terminal hairs, a sebaceous gland and duct, and an arrector pili muscle. Given an average of 2 hairs per unit, that works out to around 625 follicular units per square inch of scalp. How Does the FUT Hair Transplant Work?

The first step in the hair transplant process is to clip the donor area, generally from the back of the scalp in the thickest permanent hair bearing zone, , to a specific length. Because only the strip of hair that is to be used is clipped , the pre-existing hair will camouflage where the hair will be taken from. This is a big advantage of FUT, as opposed to FUE where usually the majority of the back of the scalp is shaved, which does not allow for camouflage of where the grafts are taken from. Then, both this area and the recipient area are anesthetized. Once the anesthesia has taken effect, a special scalpel will be used to cut an ellipse of hair-bearing scalp from the donor area. That ellipse will be carefully slivered into smaller pieces of hair, which are then further partitioned into single or double follicular units using a stereoscopic microscope. During the removal of the strip and the dissecting of the strip magnification loupes are used by the physician and all dissecting is done with the use of microscopes. FUT affords the least amount of transection or damage to the follicular unit graft, in comparison to FUE where there is a slightly higher transection rate to the hair follicles since microscopic dissection is not used for FUE.

Once the strip graft has been separated into individual follicular units, Dr Paul Cotterill will create each recipient site for each unit graft. This includes ensuring the proper angle, direction, depth in incision, and spacing between grafts.

This article offers a general timeline for hair transplant recovery and what you can expect at all stages.

Day 1

On the day of your hair transplant , your surgeon will extract hair follicles from areas of your head where hair growth is robust, usually the back or side, and insert them into the parts of your scalp where you are experiencing hair loss. Performing an FUT procedure ( whereby a strip of scalp is removed and the wound sutured closed ), allows the hair at the back of the scalp to cover the sutures. That way the excision line will be camouflaged. There can still be lingering effects of anesthesia at the end of the procedure , so it is advisable to have someone assist you home.

You should also make sure you eat something substantial after your surgery and take any antibiotics as directed.

Avoid drinking alcohol on the day of the surgery and while taking any medications. You should also avoid exercise, such as lifting anything over 10 lbs. Do not bend your head below your waist or even bend your head to read.

Day 2

After a night’s rest, you will return to the surgeon’s office to have your scalp and grafts inspected. The day after most patients do not experience any pain although there may be the beginnings of swelling of the forehead which can take a few days to disappear., Your surgeon or their staff will provide you with tips to alleviate this.

Over-the-counter pain medication such as Advil can be used for up to a week as required, and you can use an ice pack to help reduce the swelling. DO NOT PLACE THE ICE PACK DIRECTLY ON THE GRAFTS OR STITCHES. The ice pack is to be used on the forehead only to minimize swelling coimg down into the face.

You should also be sleeping at a 45° angle for 3 days or until the swelling in the area has gone down.

Shampoo instructions will be given on the day of surgery and you should start to shampoo twice a day starting the day after until your stitches are removed. However, please make sure you do not let water from a shower beat directly onto the grafts for 7 days.

Hair Washing Procedure

Fill the bathtub with warm water
Lay back until the water covers the stitched areas
Soak and gently massage the areas for 10 minutes
Then apply shampoo (nothing medicated) TO BOTH THE FRONT AND THE BACK OF YOUR HEAD and gently massage
Rinse well with clean water
After the stitches are removed wash your hair daily until all the crusts have fallen off

Days 3-7

Your FUT hair transplant recovery has begun! The initial pain, if there is pain, should only last around 12 hours, after which it can be managed by Advil. The swelling will slowly subside and some minor itchiness maybegin as scabs form at both the donor and recipient sites. Do not rub or scratch these scabs, or crusts, as you do not want to damage the grafts or compromise their effectiveness.

By days 5-7, your donor site incision line (where the follicles were removed) will be mostly recovered. The grafts will have taken.

Weeks 2-4

Very rarely 1 or 2 of the grafts may look red or tender, similar to an ingrown hair in the beard area. A hot compress will in most instances allow the graft to heal. Always contact your doctor if you have any concerns.

The most disconcerting aspect of this stage is the loss of hair from the transplanted site. This is normal! It occurs because the hair follicles, now transplanted and set in their new position, are resting. The transplanted hair will start to grow at 3 months , as will any shed hair at the donor site.

Weeks 2-14

As your FUT hair transplant recovery continues, hair is shed and the follicles recuperate. They usually begin to produce visible hair around the 12th week. The transplanted hair will be thin and fine, but it will become thicker and darker over the months.

Week 15 and Beyond

You and your follicles are recovered at this point, but your new hair will continue to grow and change. The new hair will be at least 1.5 inches long at 6 months and fully mature at 12 months. Be sure to attend your follow-up appointment with your surgeon so they can evaluate the results.

What to Expect

A typical transplant generally takes from 3 -5 hours. Upon arrival at the office the patient is given a tablet to relax the patient. The area to be treated is numbed with a local anesthetic, so that there is no discomfort during the procedure. Once the hair has been removed from the donor area at the back of the scalp and dissected into follicular units, which are small 1-4 hair bearing grafts, then the hair is transplanted into small sites that Dr Cotterill has made in the recipient area. During the procedure, the patient is in a semi reclined position, able to get up for breaks, watch a movie and have snacks.

Recovering from Female Hair Transplant

There are two techniques for removing hair from the donor region at the back of the scalp. One technique, called strip excision, is to remove a small strip of scalp from the back of the scalp and then stitched closed. That strip is then dissected into small 1-4 hair grafts. With the strip techniques the stitches are covered by preexisting hair and are not visible. The other technique for removing donor hair is called follicular unit excision (FUE), whereby the donor area is shaved, and 1-4 hair grafts are excised one at a time. For most woman, the strip procedure is more advantageous, because with FUE, since the back of the scalp is shaved and the donor area is visible, whereas with strip excision the donor area is completely camouflaged. Dr Cotterill will discuss the best technique for each individual patient. There can be swelling in the forehead area that can develop after a procedure that is gone in 3-7 days. The stitches are removed in 1 week. It is recommended not to exercise or do any heavy physical activities for seven days. A pain tablet will be offered to you for the night after surgery for any discomfort.

The grafted hair will initially fall out after a few weeks and go into a 3-month dormant phase. At 3 months all the grafts will start to grow and will be fully grown in by 10 -12 months after surgery. This is a gradual process and after a year your doctor will do a full evaluation.

What to Expect After Hair Transplant Surgery

The Procedure Itself Is Virtually Painless and the Recovery Period Is Fairly Quick. Here’s a Timeline of What You Can Expect:

What to Expect After Hair Transplant Surgery

The day of the surgery

You’ll be instructed to sleep with your head elevated at a 45-degree angle to avoid rubbing and dislodging your new grafts. You can also apply ice packs to avoid swelling.

The next day

You’ll come back to the office so Dr. Cotterill can assess your grafts and so you can have your hair washed. After this, you’ll be allowed to wash your hair gently once a day with a special shampoo. At this point, you can also return to work. However, you may wish to take additional time off, as scabbing will be visible on the transplant site for another six days or so.

Second and third day

You’ll begin to notice some scabbing and crusting and your scalp will begin to swell slightly. Continue sleeping with your head elevated and applying ice packs to bring down swelling.

One week later

You’ll be able to return to your normal exercise routine. Around this time, you’ll come back to the doctor’s office to have the stitches from the donor area removed. Any swelling should be gone by now.

Eight to 10 days later

Your hair grafts will now be firmly rooted in the scalp and the danger of dislodging them will be gone. If there’s still scabbing, it should be gone by the 12th day.

Two to three weeks later

Don’t be surprised when the newly grafted hair begins to fall out. This is expected and normal. You may also experience some minor shock loss, in which non-transplanted hair falls out and go into a typical 3 month dormant phase.

One month later

You can have your hair cut at this point. Your hairline will look much like it did before your surgery, because the hair follicles are still dormant.

Three months later

The grafted hair will begin to grow. If you experienced shock loss, this hair will also begin to grow back.

Six months later

The hair will be about an inch-and-a-half long, but its texture will be thinner and finer than your regular hair.

One year later

You’ll have full hair growth at this point. Your hair’s thickness and texture will be at its new normal.

Get the Facts About Low Level Light (Lll) Lasers

Low level light lasers (LLL lasers) for scalp hair loss have been on the market for many years. Lasers are very safe and have FDA approval for use in scalp hair loss. However, the results and overall effectiveness of LLL lasers to date have been inconsistent.

LLL lasers can come in the form of brushes or combs that have from 5 to 9 individual laser lights per comb or up to 107 lights in bonnets that are used in a doctor’s office. I am currently involved in a clinical study to test a LLL laser for male and female pattern hair loss. This laser has the potential to be 1 of the more efficient lasers at promoting hair growth and slowing hair loss (due to the increased number of laser lights), while at the same time being very user friendly. 224 laser lights are affixed to a mesh framework that fits inside a man’s baseball cap or woman’s hat. In this format the patient can perform scalp treatments discreetly at home and is not tied to a doctor’s office. I am currently recruiting patients to be involved in clinical studies. Dr. Cotterill’s Report from the Hair Surgery Meeting in Amsterdam

I returned recently from the biggest annual hair surgery meeting of the year, where I was an invited speaker and moderator. In my last blog I posted that I would report on the status of some new research going on:

Stem Cell Multiplication

This is where permanent cells from a patient’s own hair follicle are extracted and then expanded/multiplied in cell culture and then injected back into the patient’s own scalp, at which time a new hair follicle is induced to form. This has the potential to be very exciting for people with extensive hair loss and little of their own hair to transplant. The added benefit would be an unlimited amount of a patient’s own permanent hair taken from only a single extraction site. Unfortunately the predictability of this technique is not consistent. As a result, the companies that are doing the clinical trials are looking at new ways of inducing cellular growth in a manner that is more predictable, consistent and reproducible. Much more work needs to be done before this technique becomes main stream.

The consensus on low level light lasers that are being used to stimulate hair re-growth and improve existing hair showed that there is no consensus on long term multi-centered regulated studies yet to indicate how often lasers should be used, at what intervals, and at what wave lengths. More and more reports are offering the potential benefits but third party unbiased studies still need to be presented.

Current research on females with female pattern hair loss is pointing to a potential immune-based cause that could trigger inflammation with resulting scalp hair loss. More studies needed to be completed but there may be a place for treatments in women using topical anti-inflammatories and low level light lasers.

Bimatoprost, a prostagladin analogue, used in the treatment of glaucoma has recently received FDA approval in the Unites States as a medication to increase the length, density and pigmentation of eyelashes. This medication will likely be approved soon in Canada. Research is underway to assess the influence of prostaglandin receptors on the scalp at improving scalp hair. This could have positive benefits in the future and could be quite exciting.

I will be attending the Annual ISHRS Meeting in Amsterdam. This is the biggest international meeting of the year devoted to the latest news in hair surgery, hair loss and research. I am an invited faculty moderator as well as faculty for the Breakfast with the Experts session.

Some topics of interest to my patients will be discussions on the present state of investigations on stem cell multiplication/hair cloning; the use of lasers to promote hair re-growth; genotyping research to determine who with hair loss may respond to medical anti-androgen treatments; the use of a medication that can help to grow eyelashes to make them longer and darker; and the use of a patient’s own blood to potentially improve growth of transplanted hair.
